About Personal Information

What is Personal Information?

Personal information means any information about an individual which can identify the specific individual by name, date of birth and other description contained in such information （including such information that can be easily collated with other information and enables the identification of the specific individual.） Personal data items to be collected by our center when enrolling in clinical trials are:
  1. Name
  2. Date of birth
  3. Address
  4. Telephone number
  5. E-mail address
  6. School or company
  7. Bank account number to which cooperation fees will be transferred
  8. Medical history, history of present illness
  9. Result of medical check-up
  10. History of enrollment in clinical trials

Management of personal information

At this center, personal information provided by you is properly managed in full compliance with laws, regulations and internal rules concerning the handling of personal information. We strive to take appropriate and reasonable safeguards to protect your personal information from unauthorized access, loss, destruction, falsification and leakage. In case such incident occurs, we will promptly take remedial actions.

Handling of Personal Information

The Institute will use personal information provided by you for the following purposes:

  1. Application to the Ministry of Health, Labour and Welfare
  2. Checking with Japan Association of Contract Institutes for Clinical Pharmacology （JACIC）
      　   Items to be checked: name, date of birth, history of having received investigational drugs
      　   Purpose of use: Confirmation of investigational drug administration and enrollment in trials among member institutes
      　   Scope of use: JACIC and its member institutes
  3. Determination of whether or not to allow the registration at our facility
  4. Determination on the enrollment in the clinical trial
  5. Sending information regarding the enrollment or the result of medical check-up
  6. Payment of cooperation fees
  7. Other emergency calls

Provision of personal information to third party

Our institute will not provide any of your personal information to the third parties, nor deposit your personal information to an external institution without consent of the person in question.

Disclosure, correction and deletion of personal information


You may request the disclosure of your personal information retained by our institute. Based on the result of the disclosure, you may also request our center to correct/delete your personal information. If you wish to make a request for disclosure, correction or deletion of your own personal information, please contact our administration office via e-mail. During the process, we may ask you to confirm your identity.
